How to make a claim

Sports Disability (Personal Accident)

The claim should be reported to the England Netball Affiliations Deaprtment.

The easiest way is to download the Personal Accident Claim Form and return completed  to the EN Affililations Department along with supporting documentation (see below).

Alternatively you can call 01462 442344 and ask for a claims form to be mailed to you. This should then be completed and returned with supporting documentation (see below).

Supporting Documentation

You will need to provide the following information with your claim form:

• Match report or Accident Report Form detailing the accident causing bodlily injury.
• Relevant medical records, including doctors/consultants notes advising of details of the disability.

All other types of loss

It is essential that you notify JLT Sport (0845 3372446) immediatly of any claim or potential claim. It is also important not to admit liability or discuss the matter with a third party. JLT Sport staff will guide you through the claims process.
